French appeal court allows adoption of assisted reproduction babies


Lesbian couples in France may now adopt babies born due to assisted reproductive technology (ART), the highest French appeals court ruled on Tuesday.
ART - also known as assisted reproduction - is banned in France but the fact that a mother has gone abroad to use it is “not an obstacle” to the adoption of a baby here, the Court de Cassation declared.
Some courts have ruled that such adoptions were fraudulent evasion but the court ruled against that argument.
Opponents of France’s gay marriage law, which was passed last year, have called a demonstration in a fortnight’s time against ART, claiming that it opens the way to surrogate parenthood, which they say would lead to the commercialization of the birth process.
Other campaigners, including hundreds of women who admit to having gone abroad for the procedure, have called for an end to the ban on ART in France.
Socialist MPs dropped an amendment to the same-sex marriage bill that would have legalized it during last year's massive protests.
In June the European Court of Human Rights ordered France to end a ban on recognizing children born to surrogate mothers.

For infertility treatment, should he drink less coffee, more booze?

A man's love of coffee could hamper the success of a couple's infertility treatment, a small new study suggests.
But mild alcohol use by would-be fathers might help boost the odds of pregnancy through in vitro fertilization, the findings indicate.
The Boston researchers aren't ready to encourage men enrolled in IVF to cut coffee consumption and have an extra beer with dinner. Still, these preliminary results were "definitely surprising," said study co-author Dr. Jorge Chavarro, an assistant professor of nutrition and epidemiology at Harvard School of Public Health in Boston.
"We'd like to follow up and see if it replicates in other populations, such as other couples who are trying to conceive naturally," he said.
The research is to be presented Monday at the American Society for Reproductive Medicine annual meeting in Honolulu.
Previous research into male fertility hasn't found connections between alcohol or caffeine and scientific measurements of semen quality. But Chavarro said those measurements might not reflect other aspects of male fertility that are crucial to a man's ability to impregnate a woman, he said.
Enter the new study, which looks beyond measurements of sperm quality to evaluate the effect of male beverage consumption on IVF birth rates.
The researchers focused on 105 men, average age 37, involved in 214 IVF treatments between 2007 and 2013. All answered questions about their diet before undergoing treatment.
Overall, more than half of the couples achieved a pregnancy in each IVF cycle, the researchers found. Couples in which the man consumed the most caffeine (equivalent to three or more 8-ounce cups of coffee a day) were half as likely to have a pregnancy as couples where the male consumed the least caffeine (less than a cup of coffee daily), the researchers said.
Ultimately, after adjusting their statistics for factors such as age or obesity, the researchers said live births were least likely -- just 19 percent of the time -- in the couples whose male partners consumed the most caffeine.
But the live pregnancy rate was 52 percent to 60 percent for couples where the men consumed the least caffeine, said study author Dr. Anatte Karmon, a research fellow of reproductive endocrinology and infertility at Massachusetts General Hospital.
The numbers for alcohol consumption only bordered on statistical significance, Karmon said. While 28 percent of couples in which the men drank the least alcohol had live births, she said, live births occurred for 57 percent of couples where the man drank the most -- 22 grams or more per day, which is less than the 28 grams found in two "standard" drinks or two 12-ounce beers.
Chavarro said researchers don't know why caffeine or alcohol might have these effects. One possibility is that they're making men more or less likely to have sex with their partners. But Chavarro said that a change in sexual routine probably wouldn't have had much effect on pregnancy since the couples had infertility problems.
Dr. Rebecca Sokol, a professor of medicine, obstetrics and gynecology at Keck School of Medicine at University of Southern California, questioned the study's conclusions. The researchers only looked at a small number of men and the study doesn't prove whether alcohol or caffeine actually affect fertility, she said.
Other research suggests that alcohol could be toxic to sperm, said Sokol, who is also president of the American Society for Reproductive Medicine.
What should couples do with the information from the study? Nothing right now, Chavarro said. "As far as we are aware, this is the first time this has been reported," he said. "There needs to be a lot more replication before anyone can make a strong recommendation to patients."
However, "we know from other studies that alcohol at very high intakes is not a good idea" for men in infertile couples, he said. "The alcohol levels in this study are very modest."
Sokol also advised moderation. "It is in the best interest of couples who are trying to initiate a pregnancy that they moderate their use of drugs and chemicals because all could potentially impact fertility," she said.
Chavarro is co-author of another new study being presented at the fertility meeting that suggests -- but doesn't prove -- that eating fruits and vegetables containing residue from pesticides could harm sperm quality. The study doesn't say if organic food would be a better alternative, however.
Another new study, written by researchers at Sao Paulo Federal University in Brazil and scheduled to be presented Tuesday at the conference, finds that smoking can impair fertility. Smoking contributes to problems with semen quality in men with a condition known as varicocele -- enlarged veins in the scrotum -- the researchers said.
All three studies should be considered preliminary since they have not undergone the peer review process required of research published in major medical journals.

Experimental infertility treatment seems effective, cheaper

A crucial part of conventional in vitro fertilization (IVF) -- the incubation of embryos in a laboratory dish -- can instead take place in a device inside the vagina, new research suggests.
Scientists from the United States and Colombia contend that the device, called an INVOcell, might sharply cut costs for pricey IVF procedures among certain women. It could also make the technology more accessible to those who don't live near big-city assisted reproduction centers, the researchers said.
In a small U.S. study, pregnancy rates were nearly identical between women whose eggs were conventionally incubated with sperm in the lab for five days and those whose eggs and sperm were placed in the INVOcell device for the same amount of time.
Once thought of as producing "test tube babies," IVF is one of the most prominent forms of assisted reproduction and has been used in the United States since 1981. About 65,000 babies were born in the United States in 2012 through 176,000 assisted reproduction cycles, typically costing upwards of $10,000 apiece, according to the U.S. Centers for Disease Control and Prevention.
"One reason assisted reproduction is limited is because of cost, and part of that is the incubation systems we use ... which are complex devices that require calibration and daily quality control checks," said study author Dr. Kevin Doody, founder of the Center for Assisted Reproduction in Bedford, Texas.
"We think we've been able to simplify the IVF process to require minimal monitoring with a high pregnancy rate," Doody added.
The research is scheduled for presentation Wednesday at the American Society for Reproductive Medicine meeting in Honolulu. Studies presented at scientific conferences typically have not been peer-reviewed or published, and results are considered preliminary.
Optimal candidates for the INVOcell technique are infertile women able to produce many viable eggs using ovulation-stimulating hormones, experts said.
Manufactured by INVO Bioscience of Medford, Mass., the small, cylindrical device can be held in place in the vagina with a diaphragm, where the vaginal environment is expected to maintain the right temperature, pH and other conditions necessary for fertilization and early embryo development. After three to five days, the device is removed and embryos can be recovered and transferred to the uterus.
Doody's study divided 33 infertile women between ages 18 and 38 into two groups. After egg retrieval, each patient's eggs were incubated in a petri dish with sperm for two to four hours, after which up to 10 eggs per patient were either placed in the INVOcell device or moved to the next stage of conventional lab incubation.
Embryos resulting from both methods were of comparable quality, and similar pregnancy rates resulted: 10 of 17 women from the INVOcell group and 10 of 16 from the incubator culture group reported ongoing pregnancies.
"My impression is wow, this is really interesting stuff," said Dr. Timothy Hickman, chief of reproductive endocrinology at Houston Methodist Hospital in Texas. "IVF tends to be a highly complex process ... and here's a novel way to try to provide something for a certain population that can benefit. This is never going to replace an IVF lab, but maybe for a certain population it may work out right."
A second study on the INVOcell device presented at the reproductive medicine meeting indicated that the technique is also effective when eggs need to be manually injected with sperm to achieve fertilization, a procedure called intracytoplasmic sperm injection.
Dr. Elkin Lucena, scientific director and founder of Colombian Fertility and Sterility Center in Bogota, led research on couples who underwent 172 cycles of IVF in which their embryos were incubated in the INVOcell device for three days, with an average of two embryos transferred into the uterus afterward. Pregnancy resulted in 40 percent of embryo transfers -- roughly the same rate as conventional IVF.
"Especially from a psychological impact, couples feel they're participating in conception too and carrying their own [offspring] inside of them instead of in a lab," Lucena said. "And the cost is lowered a lot, which is making it easier for people to access these techniques."
The INVOcell vaginal device, currently under review by the U.S. Food and Drug Administration, is already approved for use in Canada and several other countries.
Doody, who said he owns about $10,000 of publicly traded stock in INVO Bioscience, said he thinks using the device could halve the cost of a typical cycle of IVF treatment.
"My guess is that at least two-thirds of IVF patients could be treated with this type of [technique]," Doody said.
One difference between the conventional treatment and the INVOcell approach is that eggs and sperm are placed together in the intravaginal device before knowing if fertilization has occurred.
"That is a waiting game for the patient," said Doody. "But in all the cycles we've done, we've not had any cases where we didn't have embryos [created] to transfer back to the uterus," he added.

Yesterday I received this email, the subject line of which read My spiritual crisis.
Hi John! I am writing you to get your insight on a struggle I am having. I grew up in a Southern Baptist Church, and loved it. I had what most Christians would call a very close “relationship” with Jesus. Then I went to college. I started questioning things and decided that some of the things I had been taught just were not right. I became extremely more liberal in my views, but still very much involved in my Church and “strong” in my relationship with Christ.
Then I got married and tried for three long years to get pregnant. Three long years of painful, expensive, invasive fertility treatments. I can not even begin to explain to you the emotional sorrow that infertility caused me. It also caused me a huge spiritual crisis. How can an all-loving God bless drug addicts with babies that they don’t even want, while I sat crying with empty arms? Had I not been a good and faithful follower pretty much all my life? Why was I being passed up on the blessing of growing a family? I could not, and still do not understand it.
I eventually found a medication that worked for me, and was able to conceive a son. He is now three years old. For a while the joy of having him overshadowed what I had been through, but now it has been another three years of infertility, and I am going through it all over again to have another child. Only this time, it is worse in a couple of ways. This time, we have no insurance coverage for infertility and no money to afford treatment, so I am left feeling even more helpless than before. Worse than that, however, is that this time, I know firsthand what I am missing out on, so it seems to hurt even more if that is possible. Every time a friend or family member shares their good news that they are pregnant, a small part of me is happy for them, but I mostly just feel the sting of disparity. God is supposed to be fair and just… but I have a really hard time seeing it the longer I live. I would greatly appreciate your thoughts on this. Thank you!

I’m uncomfortable answering your letter, because I don’t think you’ll like my response. Nontheless:
1. You did have a child. Clearly God fought you pretty hard on that, but ultimately you did receive the very blessing you desired. I’m not sure how not being granted the same amazing blessing twice serves as evidence that God is unfair and unjust. If you knew a poor man who begged, pleaded and prayed to God to bring him a million dollars, and then he got a million dollars, what would you think if he later claimed that God was unfair and unjust for not bringing him a second million dollars?
2. You believe that God has made you infertile; you believe that being a mother is God’s highest desire for you; you know that there are babies out there who need a mother at least as desperately as you want another baby. So maybe adoption might be something for you to look into?
Love to you. I know you’re suffering. But do consider the idea that if God blocks one path for you, it’s because he’s got something even better waiting for you down another.

Artificial womb - fantasy or reality?


Infertility has brought down the planet as a plague. The only difference is that infertile people do not die and for years on end suffer from mental disorders caused by inability to have own child. According to the world statistics, 20% of the world's population cannot get pregnant (having permanent sexual partner). The prevalence of primary infertility among women at the age of 20-35 years is of 6%. These figures are real and they have been increasing from year to year. Today there are solutions of male and female infertility problem. These are surrogacy on own or donor eggs, IVF with donor eggs (semen), ICSI. Using modern methods of reproductive medicine, you can become a parent of a child who will be genetically absolutely your own.
The above mentioned methods of assisted reproduction have been already managed to settle down in the global society, and every year their popularity and usage have been growing. Not all countries have legalized such kind of procedures but infertile couples are not afraid of local bans. They tooth and nail travel to countries where surrogacy and donation are allowed by the law, and successfully carry out programs there. But specialists have taken a step further. Artificial insemination was successfully justified and has a great demand in the modern society. It's time to move on. Artificial womb is “on the agenda". It is expected that in future there will not be a need for surrogate mothers services and carrying a child during 9 months.  
Tokyo researchers have successfully taken goat fetuses and put them in synthetic wombs for continued gestation! The fetuses have stayed in the environment for up to three weeks, but chairman of the Department of Obstetrics and Gynecology at Juntendo University in Tokyo, has said that this can be extended or applied to human beings! He believed that, by 2074, human pregnancy would be obsolete and artificial wombs would be the norm.
One of the positives that come out of this research is that this may be a way to care for preemie babies in the future. Micro-preemies could potentially be placed directly from a human womb to an artificial womb if the pregnancy is ending early due to complications. This could be especially beneficial for lung development in babies under 30 weeks.
It may also turn out that artificial wombs provide safer environments than natural wombs which can be invaded by drugs and alcohol from a mother's body. Health insurance companies could actually insist that women opt for the artificial way.
Geneticist of the London College of Aarathi Prasad believes that such method of reproduction may be an option not only for women suffering from infertility, but also for all women, since pregnancy and childbirth, even today, are a risk to the woman's life.

Countries of surrogacy

Surrogate motherhood has its origins in ancient times. In those times infertile couples also asked women to give birth and give them a child but people did not call this process surrogacy. Nevertheless since the birth of civilization to the present times children are considered to be a continuation of a single race and humanity in general. If children are no longer born, mankind will gradually die out and the Earth planet will become an empty field with a finished game.
Today, we can see that along with the rapid development of countries diseases that attack the human race rapidly are not way behind as well. Infertility is an illness which with certainty can be called plague of the 21st century. This disease affects both men and women due to the bad environmental influence on health, unhealthy lifestyle, bad habits and many others. But people are not willing to put up with such state of affairs. Surrogacy is the most popular method of reproductive medicine which helps infertile couples to feel all the happiness of motherhood and fatherhood.
According to statistics, every sixth couple in the world knows firsthand about infertility. But having such method as surrogacy in medical arsenal not all countries agree to enact it on a legal level. As of today, only a few countries have legalized this method of reproductive medicine.
India has managed to enter the list of surrogacy leaders. After studying Indian law concerning surrogacy one can see that there are no particular limitations: unmarried men or women, couples who registered their marriage or those who live in a civil marriage can choose surrogacy program. As of the price, such procedure costs about 20 thousand dollars, which is much less than in any other European country. But, take your time and do not agree with such an ideal proposition. As surrogacy in India has both pros and corns. In fact, as it turned out the whole process of surrogacy occurs in conditions that are far from norms and standards. Not always surrogate mothers go through all needed medical diagnosis and tests. Courageous women who have agreed to participate in such procedure shoot Niagara only because of a critical shortage of money. They live in terrible insanitary conditions (including period of pregnancy). It can be said that in India, surrogacy motherhood is a kind of product on the fertility market. And quality of this product is not controlled appropriately. Do not also forget about the non-standard Indian law with concerns ultrasound diagnostic. In India, it is forbidden! This is due to sexual character issue. According to the old tradition boys are considered as a future heirs and breadwinners, while girls are just a burden. That is why for a long time after the ultrasound procedure women made abortions if baby's gender was female. Therefore, in order to prevent selective abortions authorities banned the ultrasound investigation. Therefore biological parents of the child risk of getting ill and sometimes with serious disabilities or physical defects child.
Ukraine has no such defects in national legislation. Ukraine is called the third world country but conditions there are good. Clinics of reproductive medicine are equipped with modern equipment, surrogate mothers are tested according to common norms and in addition one can find centers where patients are be offered the "all inclusive" package and 100% guarantee of a positive program’s outcome. And as of Ukrainian doctors, from hearsay of patients they just work wonders in the field of reproductive medicine. Nevertheless some shortcomings also were found. Ukraine is a country which the majority of Europeans are afraid to go especially with the aim to use methods of reproductive medicine. Ukraine is behind in the European development in general. Visiting this state foreigners have the impression that they got in a time machine and flew many years ago. Bumpy roads that make impression you get into a turbulence zone, public transport that extremely needs replacement due to their old age and not always a European quality of services. The most loyal legislation concerning surrogacy is just in this state of Eastern Europe. It is because of the legality of this procedure and law prices many Europeans visit Ukraine to take advantage of a surrogate motherhood. But as a result of the massive influx of patients it can be seen a shortage of surrogate mothers. In addition, turn to join the program sometimes can last for 2 years.
Of course, comparing the level of development in the field of economy, infrastructure and medicine of Ukraine and Israel Ukraine is lag behind in many positions. Israeli medicine is known as the best one among world leaders based on good results and high quality of treatment of a wide range of diseases. Reproductive medicine, in particular its assisted methods also do not keep in the background. Surrogate motherhood, which is so popular all over the world is banned in most European countries and legalized in Israel. Programs of egg donation and surrogacy are conducted here legally (Israeli law does not ban such procedures) and have a good result indicators. Nevertheless Israeli citizens massively go to Ukraine to use the same methods of infertility treatment they can use in Israel. The question that has to be answered is why? First of all due to the price. In Israel you'll have to spend a good deal of money for good service and level of healthcare. Also it is complex and long-term process to find a donor in Israel if it is needed for the program. Couple can wait for a suitable donor for months. The same situation can be seen with surrogate mothers. It’s difficult to find a woman who will agree to become a surrogate mother therefore couple can wait for years to start the program.
There are also countries where surrogacy as a solution of infertility problem is not regulated by law and thus easy exists in the state. These countries are Finland, Belgium, Ireland and Greece. Visiting any of these countries you can certainly take advantage of surrogacy program but no one will give you even the slightest guarantee that everything will be as it should be and you will be given the child after his birth. Since there is no a law which concerns such procedures therefore it is impossible to protect any rights or to prove anything. You risk facing with blackmail, frauds and a lot of additional troubles. Absence of a law that would regulate this issue generates disorganization and inaccuracy. Stages of the program can run not in the required time, terms and sums may vary, in the end, you can go home without a child spending a lot of money, energy and health.
Usually surrogate mothers receive a cash award for their service. In countries where this issue is regulated by the law parties sign an agreement in which all amounts and obligations of the parties are written. But there are also some countries where such programs can be carried out on a volunteer basis, namely, on a noncommercial basis, without compensation for surrogate mother. These countries are Netherlands, Australia, Canada, Denmark, Spain, and United Kingdom. Here you will find great service, a friendly meeting and face with the highest European standards. But you will not find there excellent organization and guarantee of positive result. In addition doctors in these countries often use frozen donor eggs that automatically reduces success of the program. It is known and proven that frozen biological material loses its functionality and quality that led to numerous unsuccessful attempts of IVF. Also it is impossible to check the presence or absence of frozen eggs in your program. You can only hope for a miracle and a successful outcome of the procedure. There is also a risk that surrogate mother will change her mind and refuse to give you a child. As a result you have a chance to stay both without money and child.
Surrogacy program in the Russian Federation is also rather expensive. But as Russian lawyers say law of their country does not complete at all concerning surrogacy. For example, surrogate mother has the right at the last moment to refuse to give genetic parents the child. And the law is on the side of surrogate mothers. As a result it gives them an opportunity to blackmail genetic parents and require a larger sum than it was initially agreed in the agreement. There are also many so-called intermediary agencies in Russia, which do not require state licensing. As a result market of surrogate motherhood includes a lot of scams that take customers money and disappear or do not conduct any proceedings in reality. Despite these drawbacks there are still those who will find Russia appropriate country for surrogacy program. Because country's legislation mentions anything about the possibility of using such a program by couples who are not registered their family relationships, single women and men, as well as non-traditional couples.
Surrogacy theme is on the peak of popularity today. More and more people want to use this method to fight against childlessness. In turn such situation forces governments around the world to reconsider the relevant bills, sometimes simplifying and often - toughening conditions for both biological parents and surrogate mothers. But there is surrogacy in the world and this is a clear fact! And those who need it can only make the right choice of the country for its fulfillment. Any mother can affirm the fact that being pregnant and having a baby is not an easy work. Morning sickness, terrible back pain – and this is only beginning! During 9 months of pregnancy women became super heroines due to the fact that they can go through all the pain and difficulties. Breakfast of child from Burkina Faso, for example, may include millet porridge. In Japan, morning meal can contain rice and natto (pasty mass of soybeans). In Jamaica, children eat mushy mixture of bananas, peanut or corn flour, in New Zealand - toast with “Vegemite” (salty paste of brewer's yeast), in China – “Juke” (it is a rice porridge, gruel, covered with marinated tofu, slices of dried meat or eggs). Just do not be surprised if in Cuba, Brazil and elsewhere in Latin America, you will see very small children sipping coffee with milk in the morning. In Pakistan, children often drink milk with ru-afza - bright red syrup of fruits, flowers and herbs. Swedish filmyolk is a mixture of dozens of varieties of dairy products, which can be tasted during breakfast in Europe, Asia, Middle East and Africa. In southern India, child can start his day with idli - a pair of cake made ​​from fermented lentil and rice.
